What You Need To Know
Our office hours are by appointment only during the following time frame:
- Monday - Friday: 9 am to 6 pm
- Saturday: 9 am to 2 pm

If you have an urgent medical need or emergency, please seek treatment at an urgent care facility or emergency room nearest you.
Lotus Physical Therapy guarantees you one-on-one treatment with a physical therapist who will be with you throughout your treatment session.
In order to provide you individualized service, we currently deliver services as an out-of-network provider. Please call our office at (631) 629-5507, and our billing specialist will verify your coverage with us.
We hope you understand that in order to give you the attention, time, and quality of care you need, we do not contract with insurance companies because their reimbursements are not congruent with the level of treatment we provide.
We are contracted with Medicare and provide in-network services for Medicare patients only.
If you have a question related to your bill or insurance, please call our office at (631) 629-5507.
You will need a referral from your medical provider to have your insurance company pay for physical therapy treatment.
We will accept an initial referral by fax or by phone from your provider, and we will obtain the needed signed prescription within days of your referral, or you may bring the signed referral form with you to our office. Please bring your doctor's prescription to your first appointment.
We often have a wait list of patients, many of whom travel from other areas to obtain our services. If you cannot make your appointment, we kindly request you give us at least 24 hours’ notice, so we can accommodate another patient.
